I certify that all statements on this application are true and complete. I understand that any false information or incomplete statements will subject me to disqualification or dismissal from any volunteer position with the Compassion Care Center or Redeemed Threads Thrift Store. I further understand that placement in a volunteer position is contingent upon background clearances (if applicable)

Volunteer Waiver and Release

I hereby acknowledge that as a volunteer for the Compassion Care Center or Redeemed Threads Thrift Store in th ecapcity of a Community Service Volunteer, I am not an employee of Compassion Connect and that I am not covered under the workers compensation plan. I intend to perform voluntary services for Compassion Connect without compensation.

As a condition of performing the above referenced duties, I hereby knowingly and unequivocally waive, release and discharge any and all rights that I, my heirs, assigns, agents or other representatives may have or which hereafter may accrue to me, to file any claim, lawsuit and/or any other cause of action against Compassion Connect , its employees, officers, agencies, other volunteers and officials as a result of preforming said volunteer services, in granting this full and complete release and waiver of liability on the part of the Compassion Connect.

I expressly desire to release Compassion Connect, its employees, officer's agencies, volunteers and officials from any financial responsibility to me for any personal injury and/or property damage I may incur as a result of my voluntary services, even when it results from the negligence, both active and passive, of Compassion and/or its employees.

I understand that accidents and injuries can arise out of my volunteer activities; knowing the risk, nevertheless, I hereby agree to assume those risks and to release and to hold harmless Compassion Connect, its employees, officers, and agencies, other volunteers and officials, who (through negligence or carelessness) might otherwise be liable to me (or my heirs, assigns, agents, or other representatives) for damages.

No promise, inducement, or agreement has been made to me to induce me to release Compassion from liability for any personal injury and/or property damage incurred by me as a result of my voluntary services, nor has any promise, inducement, or agreement been made to me in return for the express waiver of rights referred to above.

PHOTOGRAPHY RELEASE: I hereby grant Compassion Connect, aka Compassion Care Center or Redeemed Threads Thrift Store and it representatives permission to use and/or publish photographic pictures in which I or, the individual(s) named herein, may be included for promotion or other Compassion purposes. I hereby release, discharge and agree to hold harmless Compassion Connect, aka Compassion Care Center or Redeemed Threads Thrift Store and its representatives from any liability, including but not limited to, claim for liber or invasion of privacy.

The Compassion Care Center is part of Compassion Connect a faith based 501c3 non-profit agency.
