I understand the San Tan Valley Compassion Care Center is a nonprofit ministry whose purpose is to minister to individuals, families, expectant mothers, single mothers, women who are in unplanned pregnancy or having difficulty with life circumstances, and those mothers and/or fathers wanting to improve their parenting skills. I have come to the Compassion Care Center of my own free will and can ask for ministry and prayer in dealing with certain areas in my life at any time. I understand that the help provided is not a substitute for professional counseling. I understand that the client advocate/parenting instructor I am talking to is a life coach and not a counselor. I understand they are acting in the capacity of a Christian who is reaching out with Compassion and God‘s word I also understand that my information is kept confidential except when sexual or physical abuse of a minor is reported. I further understand that if I threaten physical harm to myself, my children or anyone else, it is the Compassion Care Center‘s policy to notify the police and/or department of child safety.

I understand that the Compassion Care Center is not legally responsible for a contact I have with my client advocate/parenting instructor outside of the center including but not limited to client advocate relationships  (i.e., taking clients to church, being a birth coach, babysitting or any other contact off The Center‘s promises). This is considered the client advocate/parenting instructor‘s personal time and not within the scope of the services offered by The Center.

The Compassion Care Center is not a mental or behavioral health facility. If you are suffering from depression or any other mental or behavior health conditions please direction those concerns to the behavioral health facility nearest you.

I agree that I will use the Store Bucks that I earned at the San Tan Valley Compassion Care Center to purchase items I need for my children who live with me I will not purchase items for anyone other than my children sell the items or try to return them to any store for cash, exchange for store credit. I acknowledge that the opportunity is a privilege it is not a right. This privilege may be revoked at any time. I understand that the Compassion Care Center reserves the right to limit quantities of any item. All prices are subject to change without notice.
